Our goal at Massena Memorial Hospital is to assure that you and your family feels ready for discharge. The best way to accomplish this goal is to plan for it now. Ask to speak with your Case Manager about some of these questions:
- Will I be able to care for myself at home?
- Do I need nursing care at home?
- Will I need medical equipment at home? (oxygen, walker, or wheelchair)
- Do I have a follow up appointment with my physician?
- How will I get to my appointments?
- Do I qualify for Meals on Wheels?
Every person's circumstances are different. Your case manager wants to make sure that all of your questions are answered and needs are identified before you leave the hospital.
