Important Information About the Patient Portal
- Talk to your physician about your test results — whether they are normal or abnormal. It's important for you to keep any appointments you have made with your care provider. If you don't have an appointment, please make one with your provider if you have any concerns.
- Your care provider is the most qualified person to interpret your results. MMH and your care providers are concerned about your health — please keep your appointments!

>Where does my health information in the Portal come from?
All of the information in the Portal comes from your Massena Memorial Hospital Electronic Health Record.
>How do I log into the Portal?
If you are already enrolled, click on the Log on Button above. Then, simply enter your user name and password.
>What if I am not enrolled to use the portal?
If you have not received a logon ID, you will need to stop by or call medical records. Or you can use the self enrollment button above; You will need to have your Medical Record# and the current email address you have on file with the hospital.
>Can my family access my Portal?
Yes, You will need to fill out the Proxy Request from and bring it to the Medical Records Department.
>Are my Physician office records available here?
No. You will need to contact your Physician for information regarding appointments and office visits.
>Who do I contact for possible errors in my record?

>Who do I contact to discuss my results?
Contact your primary care physician or the provider who ordered the test.
>What if my results are negative, can I cancel my follow-up appointment?
No. Always follow-up with your healthcare provider regarding any test results.
>How will I know if my results are available to view on the Portal?
You will receive an email regarding new activity on your patient portal account. Some tests are available as quickly as 72 hours after the test is performed.
>Is my information safe?
Yes. Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Portal. Also, a timeout feature protects your information if you leave the Portal page open.
>What if I ever have technical problems with the Portal?
There is a Contact Us section on the homepage of the Portal that allows you to submit any questions you have regarding the Portal or you can email firstname.lastname@example.org or you can leave a message at 315.769.4386. Please allow 1-3 business days for a response. Please feel free to contact us with any suggestions or improvement you have regarding the portal.
